Lori Chapman 57 of Louisa, KY passed away Monday Feb. 9, 2015 at the Community Hospice Care Center in Ashland, KY.
Lori was born Dec. 10, 1957 at Toledo, Ohio the daughter of the late Alma Chambers. She was a homemaker.
Survivors include her husband Donny Chapman of Louisa, KY; two daughters Alicia and Angie Chapman both of Ashland, KY; 3 grandchildren.
Graveside rites will be held on Friday Feb. 13th at 11:00 am at the Yatesville Memorial Gardens with Rev. Jim Smedley officiating.
Care has been entrusted to Wilson Funeral Home.
